COMMUNITY HEALTH OUTREACH WORKER

 

The City of Portland’s Health & Human Services Department is hiring a Community Health Promotion Specialist to provide outreach to individuals experiencing homelessness in Portland.

 

This exciting position will help community members access housing and community services and will provide connections to health and other social supports. Successful candidates will have experience working with unhoused and underserved populations in the Greater Portland area. We are looking for individuals with a passion to support vulnerable populations such as the unhoused and those experiencing mental health and/or substance use disorders.

 

Duties & Responsibilities:

 

• This position will be a pivotal role in the exciting new Encampment Crisis Response Team efforts and work collaboratively with community partners.

• Successful candidates will be able to connect with unhoused individuals and provide support to access housing, medical services, food, water, or other needs, by connecting them to the appropriate social and health services entities and community organizations who can provide these services.

• They will provide outreach and education to these individuals and serve as a liaison between the unhoused community and City of Portland programs.

• Individuals with lived experience and/or experience working directly with the unhoused community are strongly encouraged to apply.

 

Requirements:

 

• Minimum of a Baccalaureate Degree in public health, health education or related field or any equivalent combination of experience, education and training which provides the following knowledge, skills and abilities:

• Strong oral and written skills are essential.

• Ability to establish and maintain effective working relationships with community members, providers, physicians, public health staff and consumers.

• Must be able to do outreach and travel by car to encampments, agencies, and private providers.

• Ability to work with a diverse population.

• Experience using personal computers and data based software.

Salary & Benefits:

 

This is a union position, Pro-Tech(Grade 6) - Starting at $26.27hr.
