Permanent, Full Time (37.5 hours per week)
This position has two openings, available schedules are : Monday through Friday, 10am to 6:30pm & Thursday-Saturday 7AM-8:30PM.

The City of Portland is seeking a highly motivated and compassionate to join our Prevention and Diversion Team as a Human Services Counselor. In this role, you will provide support and assistance to individuals and families who are homeless or at risk of becoming homeless, with an emphasis on prevention and diversion. The ideal candidate will be flexible, look forward to taking on new projects, and can adapt to a changing population.

Duties & responsibilities include but are not limited to:

  • Conducting assessments to determine individuals' needs and develop care plans including: referrals to detox or other substance use providers, connect to mental health services, referrals to healthcare options.
  • Provide outreach services and create relationships with those who are unsheltered within the City of Portland.
  • Meet with residents to navigate possible homeless prevention assistance
  • Collaborate with City departments and community agencies to provide comprehensive services.
  • Managing open beds at the Homeless Services Center and triage appropriate referrals.
  • Problem solve with those who are homeless to find possible alternatives including friends or family reunification, financial assistance, or refer to other support programs within the community.
  • Maintain accurate and confidential records of all interactions with clients.

Qualifications:

  • Experience working with homeless or low income/at risk populations.
  • At least 2 years of experience in human services or related field.
  • Knowledge of community resources and social service agencies.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Must pass a criminal background check.
  • Valid Class C Driver’s License

Salary & Benefits:

This is a union position, Pro Tech (Grade 5) - Starting at $23.92/hr.
